Cancer family history information is often underutilized in practice. How can we systematically increase the number of moderate and high-risk individuals who receive appropriate cancer screening and referral for cancer genetics services?

Primary Aim:  To engage and enlist people and identify resources to increase referrals to cancer risk assessment and related services.

Desired Outcome:                                              

• Identification of assets/resources
• One priority objective for 2008 is chosen
• A realistic work plan to address the priority objective
• Commitment of individuals to do something
