Information and Referral Line - Hospice Minnesota
Hospice Minnesota
has an information and referral line which allows individuals to call in with
questions to find resources. Questions
often focus on:
- Advance
care directives (from where to find forms to having someone come and speak
with them personally),
- Resources
they can access as a person near end-of-life or as a caregiver, and
- Counseling to help loved ones recently diagnosed with cancer.
This activity has resulted in
materials for the public and workshops and educational sessions for professionals.
