The information requested in this form on behalf of the Minnesota Cancer Alliance will be shared with Alliance staff in order to develop a snapshot of activities underway in our state to increase colorectal cancer (CRC) screening.

This snapshot will be used to compare the range of current efforts intended to increase CRC screening rates with related Alliance strategies stated in Cancer Plan Minnesota. The goal for this comparison is to identify gaps where Alliance strategies are not being pursued fully and to create initiatives that will comprehensively implement these strategies.

Remember, this submission should reflect a project or on-going activity which is specifically Minnesota-focused, not one which has a national or international focus.

Example A: This activity is working to educate primary care doctors in Metro area clinics about the importance of CRC screening. The goal is to track screening improvement using Minnesota Community Measurement.

Example B: This is a community-based participatory research project examining barriers to CRC screening in a specific population group in Minneapolis.
